Starting June 1, 2020, all patient forms are now digital and are easy to complete from your phone, tablet or computer.

 

Patient forms are linked in your appointment confirmation and reminder emails or SMS text messages.

 

Please complete your forms prior to your visit.

Look out for:
 

• Intake/Health History Form
  required before the first visit for each service, or after significant health events

• Direct Insurance Billing Form
  required for clinic to direct bill to extended health benefits; credit card required to opt in to program; only required once unless there has been a change in benefits providers

• Re-Assessment Form
  if have not been treated in our clinic for 6+ months, or after significant health events

• Postpartum Form
  after childbirth

• Orthotics Form
  before a gait assessment appointment

• Progress Exam Survey
  after you have completed a round of care, or after significant health events
